Children and Family Health Devon (CFHD) is delighted to advertise for a Band 5 Mental Health Practitioner to join our Assertive Outreach Team, part of the Urgent Care Pathway.

The Assertive Outreach Team works with children and young people who are at risk of hospital admission or stepping up into more intensive services, offering intensive community support to reduce crisis and promote emotional recovery. This is an opportunity to be part of a small, dynamic and trauma-informed team offering high-quality, evidence-based care that keeps young people safely connected to their families and communities.

The service operates between 9am and 8pm Monday to Friday, and 9am to 5pm on weekends and bank holidays, offering flexibility and genuine teamwork to meet the needs of the children we support.

Main duties of the job

Provide mental health assessments and risk evaluations in community settings

Deliver structured interventions and outreach care as part of a multi-disciplinary team

Work collaboratively with young people, families and professionals to reduce risk and maintain community support

Contribute to care planning, documentation, and safeguarding processes

Support a recovery-focused, person-centred approach to care

Participate in clinical meetings, supervision, and reflective practice

CFHD works closely with our localuniversities of Plymouth, Exeter & the Peninsula Medical School where wesupport students in clinical training placements and in addition support ourown staff to undertake post graduate training.

Person Specification
Qualifications
  • A professional qualification at degree level or equivalent e.g. Nursing, Social Work, Occupational Therapy.
  • Professional registration/accreditation with a recognised body, e.g. NMC, HCPC, Social Work England.
  • Q3 It will be an essential requirement for newly qualified social workers to demonstrate that they have completed The Assessed and Supported Year in Employment (ASYE) before commencing in post or there will be a requirement to complete this within the first 12 months of employment with the Trust.
  • Evidence of relevant continuing professional development
Experience
  • Post graduate experience working within mental health environments or willingness to complete the DPT preceptorship programme
  • Experience managing a caseload of clients with a range of mental health problems
  • Demonstrates sound theoretical knowledge of children and young people mental health practice
  • Working within a community setting in either the statutory or voluntary sector.
  • Experience of working in a multi-disciplinary and multi-agency environment.
