Job summary

NHS Professionals is recruiting for a Band 6 Mental Health Practitioner in Worcester at Herefordshire and Worcestershire Health Trust. The role involves assessments, care planning, and coordinating support for individuals with complex needs, within a multidisciplinary team.

The position requires a degree in Mental Health Nursing, Occupational Therapy, or Social Work, plus community mental health experience. Full driving license and vehicle access are needed.

Qualifications

• Degree-level qualification in Mental Health Nursing, Occupational Therapy or Social Work.
• Experience of care work within a community mental health setting.
• Full driving license and access to a vehicle.

Responsibilities

• Undertake comprehensive mental health assessments under the supervision of senior clinicians.
• Develop, implement and review person-centred care plans.
• Deliver brief evidence-based interventions appropriate to the individual's needs and within the scope of professional competence.
• Monitor service users' progress and evaluate outcomes of interventions.
• Identify, assess and manage risks, escalating concerns appropriately in line with Trust policies.
• Support individuals experiencing emotional distress, promoting recovery and resilience.
• Work collaboratively with carers, families and partner agencies to support recovery-focused care.
• Contribute to crisis prevention through proactive assessment and intervention.
• Refer and signpost individuals to specialist services, community resources and voluntary sector organisations as appropriate.

Shift Patterns: 37.5 Hours, 9:00am-17:00pm, Monday - Friday

What you'll need:

• Appropriate degree level qualification, or equivalent, in Mental Health Nursing, Occupational Therapy or Social Work.
• Experience of care work within an appropriate community mental health setting.
• Knowledge of current mental health issues and legislation.
• Excellent communication skills, written and verbal.
• Full drivers license and access to a vehicle required.
