Our Crisis Resolution and Home Treatment Team provides outstanding services across Stoke and Staffordshire, ensuring high standards of care from first contact and in crisis. The Crisis Care Centre is a vital part of the urgent care pathway, offering an all-age single point of access to mental health services in North Staffordshire, including a 24/7 crisis service and intensive home treatment as alternatives to inpatient admission.
Aligned with the NHS Long Term Plan, the Centre will expand its services to support individuals in crisis via NHS 111 referrals, alongside dedicated lines for ambulance, police, GPs, and other professionals requiring urgent mental health responses.
The post-holder will help set and maintain standards of care and ensure effective coordination of the service, providing a safe and effective response to individuals presenting in crisis with acute mental health needs.
You will coordinate care with the multidisciplinary team to ensure a collaborative, recovery-focused approach to patient care.
