A leading mental health service provider in Manchester is seeking an enthusiastic mental health practitioner to join their nurse-led team. You will manage a diverse caseload, conduct assessments, and collaborate with multi-agency services. We are looking for an enthusiastic mental health practitioner to join our small, friendly, nurse-led team.
You will require experience working in a mental health setting and awareness of community mental health care delivery.
Your responsibilities will include managing a caseload of clients, conducting assessments, care planning, signposting, brief interventions, and safe discharges. The role also involves liaising with multi-agency services.
The team serves an ethnically and culturally diverse area, providing an opportunity to work with clients from various backgrounds and with diverse psychosocial needs. You will assess client and carer needs and connect them with community resources and local provider services.
We support staff in achieving a healthy work-life balance and maintaining wellbeing, offering flexible working arrangements.
Your duties will include assessments, care planning, brief interventions, safeguarding, and care coordination. We collaborate closely with probation, approved premises, and Greater Manchester Police for safe risk management. Occasionally, the role involves liaising with partner agencies to support vulnerabilities and mental health concerns, including mental health partners.
