Job description

We’re launching an innovative community care centre inspired by world-class models like the Trieste Mental Health Community Network, Navigo in Grimsby, and Clubhouse models in New York and Brixton.

We have an exciting and rare opportunity to be part of a new community mental health pilot. This centre will hold a major national profile with NHS England, which hopes that the pilot will radically transform how mental health care is delivered in the UK.

Responsibilities include:

  • Supporting the team with overall management of referrals, providing screening, triage, and trusted assessments for patients with a range of mental disorders.
  • Collaborating with partner agencies and the multidisciplinary team (MDT) to provide interventions that support service users to stay well within their local communities. Using evidence-based tools to inform care planning and support engagement with treatment and appropriate services.
  • Supporting family and carers with risk management and escalating concerns as required, ensuring clear communication of risks to all involved in care.
  • Working collaboratively within community services using shared decision-making to ensure safe and effective discharge planning or onward referral, including to primary care, social prescribers, third sector partners, voluntary services, or community groups. Ensuring timely sharing of all planning with involved parties.
  • Participating in MDT governance, development, and clinical meetings.

Essential knowledge includes mental health law, including legislation relevant to the community sector such as the Community Care Act (2015) and treatment legislation.

Additional responsibilities involve providing low-intensity CBT and mindfulness interventions under supervision, as well as psychoeducation to service users.

South London and Maudsley NHS Foundation Trust (SLaM) offers a wide range of NHS mental health services and substance misuse services. We work closely with the IoPPN, King's College London, and are part of King's Health Partners. We employ around 5,000 staff serving a population of 1.1 million, with over 230 services including inpatient, outpatient, and community care. We treat thousands of patients annually and offer extensive training opportunities.

Please note: The role will initially be based at 62 Speedwell Street, London, SE8 4AT, with a relocation planned to 1 Heather Close, Hither Green, SE13 6UG in spring 2025.
