Job description

We’re launching an innovative community care centre inspired by world-class models like the Trieste Mental Health Community Network, Navigo in Grimsby, and Clubhouse models in New York and Brixton.

We have an exciting and rare opportunity to be part of a new community mental health pilot. This centre will have a major national profile with NHS England, which hopes that the pilot will radically transform how mental health care is delivered in the UK.

We are seeking dedicated staff to help pioneer this revolutionary approach to care.

  4. Role Responsibilities: Support the team with overall management of referrals, providing screening, triage, and trusted assessments for patients with a range of mental disorders.
  5. Collaboration: Work with partner agencies and the multidisciplinary team to provide interventions supporting service users to stay well within their communities. Use evidence-based tools to inform care planning and engage patients and carers.
  6. Risk Management: Support family and carers in risk management, escalate concerns as needed, and ensure clear communication of risks to all involved in care.
  7. Care Coordination: Collaborate with community services to ensure safe and effective discharge planning or onward referrals, including to primary care, social prescribers, third sector partners, and community groups. Ensure timely sharing of care plans.
  8. Meetings and Governance: Participate in multidisciplinary governance, development, and clinical meetings.

Candidates should have significant knowledge of Mental Health Law, including relevant legislation such as the Community Care Act (2015) and treatment legislation. The role includes providing low-intensity CBT and mindfulness interventions under supervision, as well as psychoeducation to service users.

South London and Maudsley NHS Foundation Trust (SLaM) offers the widest range of NHS mental health services in the UK, including substance misuse services. We work closely with the IoPPN at King's College London and are part of King's Health Partners. We employ around 5,000 staff and serve a local population of 1.1 million, with over 230 services including inpatient wards, outpatient, and community services. We treat approximately 5,300 inpatients annually and over 45,000 community patients across various boroughs.

Please note: The role is initially based at 62 Speedwell Street, London, SE8 4AT, with a relocation to 1 Heather Close, Hither Green, SE13 6UG in November 2025.
