Liverpool Crisis Resolution Home Treatment (CRHT) is currently looking to recruit a Band 6 practitioner to join the team.
CRHT operates on a 24-hour, 7-day-a-week basis. It offers intensive home support to service users as an alternative to hospital admission, working collaboratively with various services to support service users at home.
The post holder will work on a rotational shift basis, including weekends and nights, based on service demands.
The responsibilities include assessing care needs, developing, implementing, and evaluating care programs within the framework of Effective Care Co-ordination (ECC).
The role involves operating as part of an integrated Home Treatment Service, with flexibility to deliver care in various environments.
The team assesses and treats service users with complex needs, considering alternatives to admission where possible.
The post holder must respect and value service users’ choices, working alongside them and their carers to ensure they are central to care planning.
Participation in a 24-hour roster system, on a rotational basis, is required.
Core duties include face-to-face and specialist mental health assessments, supporting police with information sharing, advising on mental health law, delivering training and education to police colleagues, and referring to partner agencies.
