Qualifications

We are seeking enthusiastic, motivated, and hardworking individuals to join the Mental Health Assertive Outreach (AO) Service. A RMN or Registered Mental Health Nurse qualification is essential.

The AO Service is a community-based tertiary psychosis service working with people with severe and enduring mental health problems. We take referrals from CMHTs, Forensic Service, PIER, and in-patient areas. We operate a team approach providing comprehensive care packages for residents of Leicester, Leicestershire, and Rutland.

The working pattern is either Monday to Friday 9:00 am to 5:00 pm or Monday to Sunday 9:00 am to 5:00 pm.

Job Description Summary (Main Duties And Responsibilities)

Main duties of the job

The Mental Health Practitioner in the Assertive Outreach team will assess, plan, deliver, and evaluate care for service users referred to the service. This includes risk assessment and management, working within a psychosocial/family-centered framework when planning individual care packages. The role requires organizing and planning your work autonomously, managing your time effectively, and liaising closely with other agencies and disciplines.

Candidates should demonstrate a commitment to providing comprehensive care packages for people with enduring mental health problems within their own homes. Excellent assessment, risk management, organizational, and communication skills are essential, along with experience and a good working knowledge of the Care Co-ordination policy, community mental health legislation (especially CTOs), and hospital admission/discharge processes.

The successful candidate will undertake the role of Care Coordinator/Lead Professional for a caseload of service users with complex mental health problems who are difficult to engage and require intensive care packages. Developing therapeutic relationships with service users to provide support and assistance, they may be subject to a Community Treatment Order.

The role also involves administering and monitoring prescribed medication, liaising with medical staff regarding adjustments, and advising clients and carers about medications.
