Job Summary
We are pleased to offer a Full Time Band 5 Mental Health Practitioner post with the East Devon OPMH team based at St Johns Court in Exmouth.
The post holder will deliver high-quality mental health services, managing a defined caseload. The role involves providing continuous assessment and treatment to people with mental health presentations and their carers. Key responsibilities include designing and coordinating personalized treatment packages and liaising with a wide range of statutory and non-statutory agencies. The post holder will be an effective member of the multi-disciplinary team.

Main duties of the job
- Contribute to the team's engagement, including liaising with other professionals and agencies.
- Impart complex and sensitive information to patients, families, and carers regarding assessment, care planning, treatment, and review.
- Work with multi-disciplinary teams to ensure consistent, well-coordinated care.
- Collaborate with primary care teams regarding client care.
- Engage with external agencies involved in care provision, such as voluntary and private sectors, police, social services, housing, and employment.
- Participate in service user and carer groups.
- Contribute to the safe and effective day-to-day operations of the service, including handling clinical inquiries.
- Provide and receive complex, sensitive information and seek support as appropriate.
- Maintain high standards of written and verbal communication, aligned with professional documentation standards.
- Participate in clinical meetings, reviews, professional meetings, and risk assessments.
- Communicate information clearly and understandably for service users and carers, following practice standards and policies.
- Facilitate fair access to social care services and promote social inclusion.
- Actively contribute to multi-disciplinary team working.
