Therole will include completing full and comprehensive mental health assessmentfor patients on referral to this service, this includes identifying mentalhealth needs, wider social needs and risk assessment and management plans.
Weutilise the FACT model in patient care where appropriate and you will beresponsible, when shift coordinator and as a care coordinator for beingresponsive to patients under this model of care and working as part of the MDTto meet these service users needs.
Create,undertake and review comprehensive patient care plans with the input of serviceusers which promote engagement, stability, recovery and risk management. Workalongside other professionals in delivered of planned care, evaluate and reviewas appropriate.
Documentcare delivery to a high standard in line with professional standards, local andnational guidance.
Supports the person and carers to promote health andwell-being, educate around harmful behaviours and signpost to relevantservices.
Maintain professionalregistration and demonstrate commitment to statutory codes of practice andcontinued professional development requirements.
Responsible for student education when allocated tothe team and to contribute to the preceptorship and education of newlyqualified staff where appropriate.
Administration ofdepot medication in the community.

JOB SUMMARY
Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi-disciplinary team as a member of an Enhanced CMHT.
Key Result Areas:
Management of own caseload in line with C.P.A. procedures and risk management protocols.
Provision of leadership to the Enhanced Team in respect of psycho-social interventions to people and their carers in line with national and local practice standards e.g. NICE
Professional:
1. Assesses needs and identifies problems relevant to the care of people referred.
Administrative:
1. Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
2. Participates in the formulation of policies at unit level.
3. Maintains accurate mileage records.
4. Manages caseload with due economy of cost and time.
5. Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).
6. Responsible for the ordering of stock with due economy.
7. Produces monthly statistics in line with trust directives.
8. Keeps the Management Team and colleagues informed of developments relevant to the area of responsibility.
Education:
1. Supervises students and trainees on placement.
