Qualifications

• Registered Pharmacist with GPhC and current UK registration.
• Demonstrated commitment to CPD and professional development.
• Awareness of legislation and NHS pharmacy practice standards.
• Prescriber qualification or working towards it.

Responsibilities

• Lead specialist clinical pharmacy services within community mental health teams.
• Prescribe, review and monitor medicines to optimise treatment outcomes.
• Design and implement individualised pharmacological treatment plans.
• Provide expert medicines advice to patients, carers and healthcare professionals.
• Support safe prescribing and medicines optimisation and governance.
