Your responsibilities include:

  1. Coordinating the care of service users by conducting assessment, planning, implementation, and evaluation of their care in accordance with the care programme approach, under the supervision of the line manager.
  2. Ensuring that the care plan is developed to meet the individual’s assessed needs and that care is delivered as outlined in the plan by qualified staff.
  3. Liaising with family members, carers, and significant others during assessment and care plan development.
  4. Developing a risk management plan for service users, incorporating contributions from the team, and ensuring all relevant parties are aware and acting accordingly.
  5. Directing unqualified staff to ensure the delivery of high-quality care to service users.
