Trinity Health Group PrimaryCare Network (PCN) is a collaboration of GP practices across Wakefield, servinga population of approximately 55,000 patients. As part of our commitment toimproving the quality of patient care, reducing health inequalities, andsupporting the NHS Long Term Plan, we are expanding our team to further developintegrated, person-centred services.

We are seeking a RegisteredMental Health Nurse to join our established and supportive Mental Healthteam. You will provide high-quality mental health nursing care, offeringadvice, support, and signposting to patients experiencing poor mental health.Working collaboratively with GPs and the wider primary care team, you will playa vital role in strengthening community resilience and delivering coordinatedcare across our four member practices.

Trinity PCN values theessential role of Mental Health Nurses as part of a wider multi-disciplinaryteam that includes Care Coordinators, Health and Wellbeing Coaches, SocialPrescribing Link Workers, Paramedics, Pharmacists, Pharmacy Technicians, and othersacross the voluntary, community, and statutory sectors, including the ICB.

Main duties of the job

Mental HealthNurses in primary care complement the role of the GP and other HealthProfessionals as integral members of the multidisciplinary team. To make thebest use of clinical skills within the practice in order to promote well-beingand maximise health gain for patients. Provide a safe, high quality andaccessible treatment room service to the practice population. The successfulcandidate with be working alongside our existing Mental Health NursePractitioner to provide a much needed and well-utilised service for ourlocality.

This role is hosted by ConexusHealthcare, the GP Confederation for Wakefield District, which supportslocal PCNs by employing staff and providing operational and clinical support.While you will be employed by Conexus, your day-to-day work will be PCN-basedunder the direction of the PCN Business Manager and Clinical Directors.

Qualifications
  • Registered Mental Health Nurse Diploma in Mental Health Nursing or Degree/BSc in Mental Health Nursing.
