A healthcare provider is seeking a Band 7 Mental Health Nurse Practitioner to join their Older Adults Outreach Team in Tamworth. This role requires strong assessment and care management skills, focusing on the well-being of older adults and facilitating their needs. The successful candidate will have a non-medical prescribing qualification and work independently across the South Staffordshire area. We are offering an exciting opportunity for a Band 7 Mental Health Nurse Practitioner to join our Older Adults Outreach Team based at Park House in Tamworth, travelling across South Staffordshire.
You will be pivotal in the continued growth and direction of the role and service whilst working with physical and social care colleagues, along with our GP and primary care colleagues.
You will have the in-depth knowledge and skills to enable you to work in the older adults and dementia speciality. You will provide indirect care through the functions of educator and advice giver to relatives, carers, and other organisations where appropriate.
The Older Adults and Outreach Team optimise the physical, spiritual, and psychological well-being of all service users referred and facilitate timely discharge, with seamless transfer to other providers or specific inpatient care. You must hold a non-medical prescribing qualification.
The client base can be unpredictable and at times challenging, but rewarding, and we will provide you with a supportive environment in which to fully utilise and develop your skills and knowledge in working in a challenging community setting as part of a truly holistic system of health and social care.
This role is Monday-Friday, and you will be required to travel independently across the south Staffordshire region. As a Band 7 Mental Health Nurse Practitioner, you will receive patients with undiagnosed problems and make an assessment of their health care needs, based on highly developed nursing knowledge and skills, including physical examination skills.
