Overview
JOB PURPOSE: The post holder will deliver high quality comprehensive mental health services. The role involves managing a defined caseload with a focus on providing expert assessment and treatment for people with complex mental health difficulties and their carers. This will include recovery co-ordination (design and co-ordination of treatment packages). The post holder will contribute to the clinical leadership of the team, providing supervision for other members and participating in team and service development, and evaluation.
Responsibilities
- Communication and Working Relationship Skills: Contribute to the teams engagement with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies. Impart highly complex and sensitive information to patients and families/carers regarding reassessment, care planning, treatment and review; work with multi-disciplinary teams to ensure consistent, well-coordinated care; liaise with primary care teams and external agencies involved in care (e.g., voluntary and private sector, police, social services, housing, employment); engage service user/carer groups; contribute to safe and effective day-to-day operations of the service including handling clinical inquiries.
- Provide and receive information that may be contentious, sensitive or highly complex to individuals and their families/carers, where barriers to understanding may exist. Maintain high standards of written and verbal communication aligned with professional documentation standards. Participate in and chair clinical meetings, including CPA and non-CPA reviews, professional meetings and clinical risk meetings. Communicate in an empathic and reassuring manner while using negotiating techniques to gain cooperation from highly complex individuals. Ensure information is communicated clearly and accessibly for service users and carers in line with practice standards and operational policies.
- Facilitate fair access to social care services and promote social inclusion for all, regardless of age, ethnicity, gender, sexual orientation or disability.
- Analytical and Judgement Skills: Undertake complex risk assessments and develop risk formulations leading to risk management plans. Conduct and continuously re-evaluate complex bio-psycho-social assessments to develop working formulations and personalised care packages in line with evidence-based practice and payment by results. Ensure ongoing re-evaluation of needs and clinical risk, using professional judgement to formulate appropriate clinical plans. Reflect and critically appraise own performance and maintain professional boundaries.
- Planning and Organisational Skills: Organise and facilitate complex meetings, including CPA and non-CPA professional and risk meetings. Manage own workload to meet the needs of individuals using services while considering wider team needs. Plan and coordinate individuals’ care packages and reviews within required time frames (e.g., Section 117, Funding Panels, CPA). Plan own workload and that of junior colleagues to ensure high-quality, cost-efficient care, and prioritise as needed.
- Patient/Client Care, Treatment and Therapy: Ensure caseload is well managed and care/treatment aligns with practice standards and recovery co-ordination (CPA and non-CPA). Understand social factors impacting mental health and develop care packages with evaluation of outcomes. Deliver services using a recovery and strengths-based approach, placing individuals and their carers at the centre and actively involving them. Deliver care in line with evidence-based practice and employ appropriate engagement and therapeutic skills; triage and signpost to the most appropriate services. Provide specialist advice to referrers/other multidisciplinary staff and enable engagement of individuals and their carers in service development.
- Safeguarding and Holistic Care: Engage with safeguarding adults and children as required; co-ordinate and chair safeguarding meetings and implement relevant care plans. Ensure assessment and treatment plans are holistic, outcome-focused and consider physical health and social needs, including input from significant others. Monitor mental health medication in relation to physical health interactions. Ensure timely and accurate documentation of clinical activity in line with guidance and best practice.
- Policy, Governance and Service Development: Participate in audits and research within the team and implement findings. Contribute to clinical governance to ensure service quality and meet performance targets. Contribute to local system development and ensure compliance with Trust policies and CQC standards.
- Finance, Equipment and Other Resources: Ensure care is cost-effective and high quality; order/monitor and maintain stock in liaison with the Team Manager. Undertake petty cash management as per policy.
- Human Resources: Provide day-to-day and clinical supervision, participate in appraisal processes, and contribute to induction of new staff and learners. Ensure core training is maintained and aligned with the team’s Workforce Plan, supporting delegated tasks with appropriately skilled staff.
- Information Resources and Administrative Duties: Use information resources to promote continuous improvement; share information securely with stakeholders; maintain accurate patient treatment records and carry out related administrative duties.
Physical Demands
The post holder may need to travel to engage with people in their own residence or hospital settings while planning discharge. Some periods may require prolonged sitting to compile complex reports.
