If so, South Shropshire Mental Health Service has an opportunity for a fixed-term Band 6 Nurse to join the team until May 2027. The post holder will work from the Bridgnorth team base and cover across the South East of Shropshire.
The post offers the successful applicant the ability to learn the skills of managing a community caseload with support from the wider MDT. The role also offers the opportunity to learn how to assess a person's mental health needs, make holistic plans of care involving the service user and their family/carers, and evaluate the care at regular intervals to help the person work towards Recovery. The role also involves the ability to learn the skills of managing day-to-day calls to the team via the Duty system and making clinical judgements on the correct pathway for a person's needs.

You will be responsible for working in an identified adult community mental health team, with the flexibility of working across localities, dependent on skill and service need.
You will be responsible for undertaking clinical assessments in order to establish the person's needs, goals, risks, and strengths in order to generate a collaborative care plan agreed with all involved. You will be responsible for delivering recovery-focused interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health-related difficulties.
You will also be expected to be available for consultation, support, and clinical supervision to other health professionals working within the team and other areas of the Trust where identified as appropriate.
