Job description

Warrington and Halton Core 24 Mental Health Practitioner

Warrington and Halton Core 24 is a specialist assessment service for service users in mental health crisis, including those referred from emergency departments and acute hospital wards. The service focuses on assessment, formulation of treatment and risk management plans, with appropriate triage and diversion from the service.

The service operates on a shift system, including long and short days (currently under review), with nights to provide a 24/7, 365-day service. Recent organisational changes have increased flexibility across the Urgent Care footprint, supporting Mersey Care's additional Core 24 services in Mersey, including Royal Liverpool, Warrington, Whiston, and Southport, where required.

Candidate Requirements
  • Experienced practitioner with extensive mental health service experience, including organic services.
  • Ability to provide high-quality, evidence-based care within the division for service users and carers in inpatient and community settings.
  • Commitment to socially inclusive practices and the recovery model.
  • Capability to act as a named nurse/care coordinator, undertaking assessments, planning, evaluation, and review of care within a recovery-focused model.
  • Active participation as a multidisciplinary team member.
  • Provide advice, expertise, and leadership within the team.
  • Supervise care for service users with complex and high-risk needs.
  • Work autonomously, making clinically appropriate decisions based on received information and team knowledge.
  • Qualified to act as a mentor for students.
