The role involves conducting assessments, developing support plans, and ensuring appropriate services and resources are accessible for individuals' needs. It requires expertise in mental health legislation and a strong understanding of the social care system. You will collaborate closely with healthcare professionals and agencies to provide optimal care and support. The role includes attending formulation meetings and working closely with our Mental Health social care locality social worker regarding clients open to services and readmitted to hospital.
