Job summary

Stockport Council is recruiting a Mental Health Community Support Worker to work across community settings with adults who have Care Act needs. The role involves home visits, community-based support, and tenancy-based support within our Care Act framework.

You will join a small team with lone-working elements, carry a caseload as a named worker, and contribute to support planning and reviews. The post is full-time, permanent, 37 hours per week, and located at Baker Street.

Your Role

This role is working as a mental health support worker within adult social care all age adults with Care Act needs. You will work as part of a team, be community based, and be providing supported to individuals within their own homes, in the community, as well as within supported tenancy settings.

There will be an element of lone working. You will have key worker responsibilities for your caseload, including support planning and reviews. You will occasionally take part on an On Call Rota and have some responsibilities for buildings whilst on shift.

About You

You will need to have a positive outlook and the ability to affect change. You will need to have knowledge of complex mental ill health and an understanding of the impact mental illness has on everyday living as well as other contributing factors. Communication and IT skills are paramount. We expect the candidate to be non-judgemental and have the ability to work towards the service user's strengths in enabling them to reach their individual potential and recovery. You will need to demonstrate an awareness of other factors that could inhibit functioning in the community and knowledge of dual diagnoses as well as the Care Act.

About Us

The Mental Health Support Work Service is a long-established part of Mental health services in Stockport. We provide bespoke packages of support to individuals who have eligible Care Act needs and are recovering from mental ill health. It is a small team, that works across a 7-day rota which involves some evening and weekend working. We operate an on-call service which is available to some service users 24 hours a day for advice and support. The Community Enablement Team works in close collaboration with the Adult Social Care Social Work teams to provide packages of care to residents of Stockport on their journey of recovery and enablement. Those in receipt of our service have a key worker and work toward their goal setting in a support plan which is reviewed regularly. The aim of the Community Enablement Team is to help people achieve their full potential whilst working towards independence.
