Job summary

West London NHS Trust is seeking an enthusiastic pharmacist to join the Local Services team in a rotational role, developing through mental health inpatient and forensic services. The post offers opportunities to work with OPMHS, CAMHS and Crisis Teams while contributing to service development and governance.

You will engage in medicines reconciliation, clinical screening, and MDT discussions, with a focus on high-quality clinical pharmacy care and ongoing CPD.

Qualifications

• Masters Degree in Pharmacy or equivalent
• General Pharmaceutical Council membership
• Postgraduate Certificate in Pharmacy Practice (Band 7); Commitment to undertake the Postgraduate Certificate in Pharmacy Practice (Band 6)

Responsibilities

• Completing Level 2 Medicines Reconciliations
• Clinical screening of medication charts
• Ensuring continuity of supply of medications
• Responding to clinical queries with medicines information and advice
• Providing medicines-related information and advice while taking part in team MDT meetings
