The role involves promoting and facilitating safe, effective, high-quality, and cost-effective prescribing and medication management within the localities and across Greater Manchester.
You will provide technical assistance within GP practices to implement and maintain high-quality, cost-effective prescribing patterns, reducing medicines-related risks to patients.
You will act as a medicines optimisation link person for specified GP practices to implement initiatives aligned with primary care and medicines optimisation priorities.
Your responsibilities include undertaking prescribing audits, supporting the development of repeat prescribing systems, managing Medicines Information enquiries, processing repeat prescriptions, reconciling medicines in patient records, and contributing to budget management and projects using prescribing data and technology.
You will work across NHS GM or designated localities on joint workstreams, promoting awareness of Medicines Optimisation issues.
Identify, implement, and deliver Medicines Optimisation and primary care quality objectives.
Work on GP practice-based prescribing initiatives to meet targets while optimizing medicines use.
Build strong relationships and collaborate with external teams.
Analyze and present prescribing data at various levels to support targeted interventions.
Develop and complete clinical and non-clinical audits to support quality improvement.
Contribute to the development of SOPs, guidelines, policies, and protocols.
Provide advice and answer queries on medicines management issues.
Carry out medication reviews in domiciliary, care home, or clinic settings, engaging with vulnerable patients and carers.

Communicate complex financial information to prescribers and practice managers regarding performance against budgets, standards, and targets.
Support and contribute to internal and external communications, including prescribing alerts and stakeholder engagement.
Demonstrate advanced computer skills using Microsoft Office applications and GP clinical systems to analyze and interpret prescribing and clinical data, supporting audits and identifying errors or opportunities for optimization.
Review, develop, and audit practice repeat prescribing systems, supporting projects like patient-led ordering.
Manage work plans for GP practices to ensure accurate delivery of Medicines Optimisation work streams.
