A leading healthcare provider in East Sussex seeks a dedicated Pharmacist for their virtual ward pharmacy service. The role involves providing patient care at home, collaborating with integrated care teams, and enhancing patient experiences through high-quality pathways. This highly specialist pharmacist post is responsible for providing oversight and direct support to the virtual ward pharmacy service provided to patients supported by CHIC (Community Health and Integrated Care) division community-based teams, including community nursing.
The post holder will work with a dedicated virtual ward pharmacy team and other members of the CHS pharmacy team. The role requires liaising with colleagues across professional and organisational boundaries in the virtual ward setting.
The post involves providing patient care in patients' homes and requires the ability to travel independently across sites. Technology will be used to support and monitor patients.
We seek an experienced, enthusiastic, self-motivated, and dynamic Pharmacist with excellent communication and interpersonal skills. The successful candidate will collaborate with colleagues across the integrated care system to develop high-quality, clinically driven pathways to enhance patient experience.
Ideally, applicants should hold a prescribing qualification and be interested in expanding their scope of practice.

This role is subject to the Rehabilitation of Offenders Act 1975, requiring a DBS check.
Salary: £53,755 to £60,504 per annum, pro-rata.
