Band 7 (1.0 WTE) + EDC (1 Year Fixed Term)

An exciting opportunity has arisen for dynamic, motivated and innovative pharmacists to join our expanding Medicines Management team at Central London Community Healthcare Trust.

We are looking for a full time Pharmacist to provide a clinical pharmacy service to our inpatients in our Bedded Services Rehabilitation wards across the Trust

CLCH is one of the largest Community Health Trusts in England and provides services across 11 CCGs to more than two million people across ten London boroughs and Hertfordshire. We are part of 4 Integrated Care Boards (ICBs). With the aim of shifting health care from Acute Hospitals to settings closer to people\'s homes, and moving away from reactive care to preventative care based on early interventions.

Main duties of the job

As an integral member of the team, you will be providing specialist expertise in the use of medicines whilst supporting to address both public health and social care needs of patients. You will be involved in conducting structured medication reviews for patients with polypharmacy, long term conditions or high risk patients ensuring an integrated approach to the management of medicines.

If you are successful, you will join the existing team of Band 7 pharmacists at the Trust covering a range of rehabilitation specialties. You will be supported by experienced Senior Clinical Pharmacists allowing you to gain new skills in each rotation and the ideal opportunity to develop strategic, quality improvement and leadership skills.

The post holder may be required to travel across the different sites within the Trust where we have Bedded Services inpatients so will require some flexibility.
