Job description

Support and assist the Prescribing Support team in Primary Care with the iLipid Project, a targeted population screening initiative aimed at enhancing the detection and management of Familial Hypercholesterolaemia (FH).

This part-time position (0.2 WTE) offers a unique opportunity to contribute to an innovative, high-impact public health initiative with potential regional and national expansion.

Responsibilities include:

  1. Contributing to the early identification, cardiovascular risk assessment, and preventative treatment of high-risk patients.
  2. Assisting in the retrospective and prospective identification of patients eligible for the iLipids test, using clinical system searches and data extraction from EMIS Web.
  3. Engaging in patient outreach through texts, calls, letters, and coordinating with interpreter services to improve access to FH screening.
  4. Working alongside a multidisciplinary team, including a Project Pharmacist and Lipid Nurse, to ensure appropriate treatments and referrals for genetic testing.
  5. Maintaining accurate documentation and patient records to support follow-up, data management, and audits.
  6. Developing and delivering training resources, workshops, and IT solutions to enhance clinician awareness and confidence in using the iLipids system.
  7. Supporting quality improvement initiatives to sustain FH screening and embed the iLipids system into routine practice.
  8. Performing other tasks as outlined in the Job Description and Person Specification for a qualified and experienced Medicines Management Pharmacy Technician.
