Job overview

King’s College Hospital is investing in the expansion of the medicines management service. We have an exciting opportunity to become a part of the medicines management pharmacy team based at our Denmark Hill site. The team delivers a high-quality service to our patients on the wards at one of London's largest and busiest teaching hospitals.

The successful applicant will be involved in medicines reconciliation, supporting stock management, operational pharmacy satellite duties, counselling patients, and working with the pharmacists to provide an efficient and accurate clinical pharmacy service to patients at ward level. In addition, you will participate in general dispensary duties, including dispensing and accuracy checking.

In order to be successful in this role, you will need to be organised, motivated, have excellent communication skills, and be able to adapt quickly to new situations. Registration as a pharmacy technician with the General Pharmaceutical Council (GPhC) is essential, as well as holding ACPT accreditation or being willing to work towards it.

Main duties of the job

• To provide a specialist patient-focused technical pharmacy service.
• To counsel patients and/or their carers on their medication.
• To assess the suitability of patients’ medicines on admission and discharge.
• To provide appropriate and sufficient supplies of medication during admission and on discharge.
• To facilitate discharge through the timely and efficient supply of medicines.
• To liaise with ward and pharmacy staff to facilitate the efficient transfer of medication and relevant information when patients move wards.
• To follow through any relevant queries arising from patient transfer or discharge.
• To liaise with pharmacists, ward staff, general practitioners, community pharmacists, and other primary healthcare professionals as required to resolve queries and ensure accurate patient medication histories are obtained, in line with local policy.
• In liaison with pharmacists, to obtain and document patient medication histories, after appropriate training and/or accreditation.
• To support with individual patient medication management issues, within own skills and expertise.

Detailed job description and main responsibilities

This role will be integral in supporting the provision of clinical services to patients on the wards. This role will involve the following:

• Ward-based dispensing services to streamline patient discharges.
• Participation in an innovative 7-day pharmacy service.
• Engaging and working in collaboration with a broad multi-disciplinary team.
• Ensuring medication storage and security criteria are met to comply with national standards.
• Gaining an underpinning knowledge and practical experience of performing medicines management duties at ward level.
• Rotating through a wide range of specialties including acute medicine, critical care, neurology, paediatrics, cardiology, renal, haematology, and surgical/gastro medicine.

Main Responsibilities

• To maintain compliance with King’s policies and procedures for Medicines Management.
• To maintain and develop high quality, consistent, patient-focused pharmacy services to wards in conjunction with clinical and patient services staff.
• To be the technical link person between wards and pharmacy, and to manage calls and requests as appropriate for the pharmacy team, via the WIFI phones.
• To propose and implement policies and new working practices for ward-based pharmacy services that will have an impact in the care group or division.
• To propose changes to working practices that will maximise the potential for efficiency, safety, and cost reduction.
• To manage and support the safe, accurate, and timely supply of medicines to wards and clinics, and other areas as required, by managing the medication supply chain in conjunction with Pharmacy team members, Stores, and Distribution staff.
• To check Patients’ Own Drugs (PODs) for use during inpatient stay and at point of discharge where necessary.
• To adhere to and maintain quality standards and performance indicators for service developments implemented.
• To cover or to participate in dispensary services, as necessary.
• To cover other specialties, as and when necessary, as required by the Clinical Services team, to the same standard, in accordance with the relevant policies and procedures.
• To support the Pharmacy Clinical Services team in the development of the medicines management pharmacy technician service.
• Be able to accuracy check clinically screened dispensed prescriptions, according to policy.
• To be able to follow departmental SOPs and dispense medication accurately.
• To be able to use pharmacy computer systems correctly, enter data accurately from transcription sheets, prescription forms, and any other prescriptions written by others on Ascribe.
• To be able to use Trust EPMA computer systems to record medication histories, order non-stock medication, endorse PODs, and Patient Own Drugs at Home assessments.
