Overview

We are seeking a Medicines' Management Pharmacy Technician to join our team at Lymington Hospital in the New Forest. The successful applicant will be someone who has abundant common sense, is a logical and practical thinker, who is reliable and able to deal with pressure. The role requires the ability to work in a team and also act independently as required.

Responsibilities
  • Promote the safe and effective utilisation of medicines in all care situations, working with care staff and patients directly.
  • Understand patient care pathways and optimise patients\' concordance with medicines.
  • Operate with a degree of autonomy to manage individual patient medicine pharmaceutical care, ensuring timely medicines reconciliation to in-patient units and community teams as appropriate in line with Trust standards to facilitate smooth admission to services.
  • Dispense inpatient, outpatient or discharge medication as required.
  • Re-supply PODs and non-ward stock items when necessary.
  • Advise patients, carers and GP staff on issues relating to medication regimes and changes to prescribing.
  • Provide patient counselling on the use of medicines to selected patients and offer information on common drug dosage and use.
Qualifications and Requirements
  • Demonstrates the ability to work within a team and independently as needed.
  • Confident in dealing with patients, carers and healthcare staff internally and with other organisations to ensure patient safety with medicines.
  • Experience or clear willingness to engage in medicines reconciliation and pharmaceutical care across care settings.
