Job summary

Cambridgeshire and Peterborough NHS Foundation Trust is seeking an experienced Medicines Management Specialist Pharmacy Technician to join the Cambridge Rehabilitation Unit team. You will support day-to-day clinical medicines management on the wards and assist with discharge planning, working closely with pharmacists and the wider multidisciplinary team.

Qualifications

• Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
• Evidence of continuing professional development (CPD).
• Post-qualification experience in hospital pharmacy.
• Experience of clinical medicines management.
• Experience of direct patient communication.

Responsibilities

• Be accountable for developing and maintaining the day-to-day specialist pharmacy technician services for Cambridge Rehabilitation Unit and Welney Ward in Ely.
• Assist the pharmacist in delivering clinical pharmacy services and refer clinical queries as needed.
• Conduct medicine histories and medicines reconciliation for new admissions and communicate issues to the care team.
• Assess patients' own medicines for suitability.
• Order medicines for ward use and discharge preparation.
• Manage stock rotation and expiry dates.
• Induct and train ward nurses and nursing associates.
• Maintain records of pharmaceutical interventions.
• Liaise with community pharmacies to ensure discharge medicines pathway.
