Overview
The Care Navigator will work with our clients to ensure that they receive the highest quality of care during their treatment, as well as provide them with all the resources and information they need, including scheduling appointments and helping to control the cost of their care. Hours of work will be 7:45am to 12:45pm and 1:30pm to 6:30pm alternate week, Monday to Friday. Pro-rata late Monday to Friday between 6:30pm–9:00pm and odd weekends between 9:00am–5:00pm.
Main duties
- To have a thorough knowledge of all Practice procedures.
- To work in accordance with written protocols.
- Ensure an effective and efficient reception service is provided to patients and any other visitors to the Practice.
- Deal with all general enquiries, explain procedures and make new and follow-up appointments.
- Using your own judgment and communication skills, ensure that patients with no prior appointment but who need urgent consultation are seen in a logical and non-disruptive manner.
- Explain Practice arrangements and formal requirements to new patients and those seeking temporary cover and ensure procedures are completed.
- Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.
- Enter requests for home visits onto the clinical system, ensuring careful recording of all relevant details and where necessary refer to Duty Doctor.
- Action repeat prescription requests and ensure that they are ready for collection by the patient within 48 hours, either on the computer or manually.
- Advise patients of relevant charges for private (non General Medical Services) services, accept payment and issue receipts for same.
- Enter patient information onto the computer as required, e.g. changes of addresses, names, telephone numbers and any ad-hoc information as appropriate.
- Ensure the waiting room is kept tidy in turn with other staff members.
- Premises: Ensure building security has thorough knowledge of doors/windows/alarm.
- Open up premises at the start of the day when first to arrive, and make all necessary preparations to receive patients.
- When last to leave at the end of the day, ensure that the building is totally secured, and rooms are tidied and ready for the next day.
- Ensure that all new patients are registered onto the computer system promptly and accurately, including Temporary Residents.
- Contact patients for routine screening as appropriate.
- Do necessary administration of Pathology Data.
- Arrange transport for patients when necessary.
- Open and stamp incoming post with date and action stamp.
- Scan hospital letters and patient correspondence in accordance with Practice guidelines.
- Advise Office Manager when stationery or other equipment is required.
- Send faxes to outside organisations when required.
- File clinical letters into relevant concertina files.
- Action tasks within a timely manner.
- Restock the reception area.
- Complete all annual Mandatory training as appropriate.
- Have working knowledge of the telephone system during and after hours.
- Undertake any other duties appropriate to the post as requested by the Partners or the Practice Manager.
- This post requires a clear DBS, good previous references and a clean employment track record.
