This role forms part of a team of MDT Co-ordinators undertaking the tracking and escalation of all patients who have been referred on a suspected cancer referral by their GP through their diagnostic pathway to a definitive diagnosis.

The post holder will be expected to work independently, but under the guidance and supervision of the MDT Lead / DOSM to set up fast track pathways from referral to either diagnosis or being discharged from the pathway.

Responsible for capturing and recording data accurately on all patients referred for further investigation from a GP referral.

Main duties of the job

  • Work independently and with minimum supervision. This will include receiving management and waiting list information and taking action to address waiting list issues.
  • Take responsibility for tracking the patient pathway, highlighting delays and escalating these to the appropriate person for action.
  • Record key patient data on the central database to strict deadlines to ensure compliance with the Cancer Standards and National Audits.
  • Take the lead role in the organisation and facilitation of Site Specific Focus Groups and ensure patient views are incorporated into service planning. This will include presenting a report on the improvements and actions identified to the Site Specific Annual General Meeting for approval and action.
  • Propose and take forward cancer service improvements including identification of new national guidance playing a key role in action planning for targets.
