Job description

Salary dependent on experience (Band 7-8A). Full time, permanent position with job share considered (37.5 hrs per week, Monday to Friday).

Main duties of the job
  1. Complete the CPPE Development Pathway.
  2. Perform structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy and care home residents.
  3. Be involved in the management of long-term conditions.
  4. Embrace service development to grow their capabilities and the clinical pharmacy team.
Candidate requirements
  • GPhC registered pharmacist.
  • Significant clinical experience; being an independent prescriber is desirable.
  • Leadership qualities and a commitment to improving patient care.
  • Ability to work effectively as part of a practice team and encourage new ways of working.
  • Experience in General Practice is advantageous, but transferable skills from other sectors are welcome.

Job responsibilities

The post holder will work within their clinical competencies as part of a multidisciplinary team to provide expertise in clinical medicines management, structured medication reviews, management of long-term conditions, medicines transfer of care, systems for safer prescribing, managing repeat prescriptions, and addressing public health and social care needs of patients in the PCN.

Person Specification
Qualifications
  • GPhC registered pharmacist.
  • Flexibility to work outside of core office hours.
  • Disclosure Barring Service (DBS).
  • Evidence of continuing professional development.
  • Hold a GPhC independent prescribing qualification.
  • Minor ailments certification.
  • Membership of the Royal Pharmaceutical Society.
Experience
  • Minimum of two years working as a pharmacist demonstrated within a practice portfolio.
  • Understanding of GPs and general practice, including primary care prescribing and strategies for improving prescribing.
  • Experience with common acute and chronic conditions in general practice.
  • In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare principles.
  • Understanding the mentorship process.
  • Awareness of the NHS landscape, including relationships between practices, PCNs, and commissioners.
