An exciting opportunity for a salaried GP maternity cover at a leading healthcare practice in Farnham. An opportunity has arisen for a 5-7 session maternity cover fixed-term salaried GP to join our team in the Georgian market town of Farnham from late October/early November, with good local schools and excellent transport links to London. As a practice, we are looking for a highly motivated, enthusiastic, and committed GP. We use a clinical triage platform called Rapid Health.
The role will be within Farnham Park Health Group. The practice is innovative, friendly, forward-thinking, and family-oriented, working collaboratively with other surgeries in Farnham. The successful applicant will share our ethos and commitment to delivering the best possible care to our patients. We have 3 Partners, 7 Salaried GPs, an Advanced Paramedic Practitioner, an excellent multi-skilled practice nurse team, a list size of 16,000, and are a training practice for Foundation Doctors, GP Trainees, and medical students. We use EMIS Web & Docman, and are part of Farnham Integrated Care Services, working in collaboration with local practices, offering same-day access, MSK Practitioners, and are part of Farnham Primary Care Network working with Clinical Pharmacists and Pharmacy Technicians. The successful applicant will share our ethos and commitment to delivering the best possible care to our patients.
In accordance with the Practice rota, the post-holder will undertake a variety of duties including surgery consultations, telephone consultations, home visits, checking and signing repeat prescriptions, and handling queries, paperwork, and correspondence promptly.
Making professional, autonomous decisions regarding presenting problems, whether self-referred or referred from other healthcare workers within the organization.
Assessing healthcare needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
Developing care plans for health in consultation with patients, in line with current practice disease management protocols.
Providing counselling and health education.
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
Recording clear and contemporaneous consultation notes using clinical system templates, Read Coding, or other standards.
Compiling and issuing computer-generated acute and repeat prescriptions.
Prescribing in accordance with the Practice prescribing formulary or generically when appropriate.
Handling NHS paperwork for own patient list and sharing responsibilities for others during absences, while building own list (results, letters, forms, scanned documents).
Proactively managing patients to meet Quality & Outcome Framework targets and other government initiatives.
Performing all normal duties associated with a GP working within primary care.
