Base Pay Range

$95,000.00/yr - $110,000.00/yr

Principal Duties / Essential Functions
  • Monitor and complete workflow tasks in HCHB
  • Code commercial OASIS
  • Organize educational programs based on needs
  • Review clinical records for completeness
  • Collaborate with supervisory staff for documentation accuracy and effectiveness
  • Lead case conferences and staff meetings
  • Educate staff on audit deficiencies
  • Evaluate clinical competency of nursing staff
  • Participate in orientation of new team members
  • Perform Blood Pressure clinics as needed
  • Provide direct skilled nursing care in the field as per physician orders
Supervisory Responsibilities
  • Serve as Home Care EMR resource with extended access and roles
  • Supervise branch staff, conduct annual reviews, and orientations
  • Train new clinicians on HCHB and homecare regulations
  • Review orders and oversee care as per licensure
Qualifications
  • Graduate of an accredited nursing or physical therapy school; BSN preferred, MSN strongly preferred; master's in physical therapy preferred; home care experience preferred
  • Minimum 3 years’ managerial experience
  • Minimum 5 years’ patient care experience
  • Current RN or Physical Therapist license in Massachusetts
  • CPR certification required
