Job Overview

Home 1st Planned Care, is a multi-disciplinary integrated service, which encompasses collaborative working by health teams with other local authority service providers, charities and voluntary organisations to support the physical and psychological therapy needs of the people of Milton Keynes. Providing admission avoidance, supported discharge and specialist therapy pathways, this role will require organisational leadership at a dynamic pace.

Main Duties
  • To provide strategic and operational management to the Home 1st Planned Care teams: Neurological Conditions Specialist Team / Neurological Rehabilitation Physiotherapy service and associated specialist pathways
  • To provide transformational, integrated leadership and service modernisation to the teams
  • To enhance the development of evidence based clinical practice and both Nursing and Allied Health Professional roles in the management of long-term conditions.
  • To provide clinical and professional leadership to the team.
  • To be aware of the national and local agendas for NHS services providing public health and preventative support.
  • To work alongside and in conjunction with the other Home 1st services to ensure the safety and wellbeing of the patients.
  • To work with the service manager and the wider health and social care economy to develop strategies in line with the National Agenda around admission avoidance and supported discharge.
  • To work with the AHP and Nursing Professional Leads for CNWL to support and promote key specialist roles within CNWL MK.

Detailed Job Description and Main Responsibilities
  • To support and guide the operational management of the H1st Planned Care Teams providing long-term conditions management, on a day-to-day basis to ensure the service can meet its commissioned targets.
  • To provide clinical and professional leadership to the team.
  • To work in conjunction with MKC, MKUH and other key stakeholders to ensure an integrated multi- agency approach to the admission avoidance, supported discharge and intermediate care pathways.
  • To ensure safe staffing levels for the team and manage recruitment and retention to support this.
  • To work with MKUH Therapy and the Integrated Discharge Hub hospital teams to develop the role of the H1st teams in enhancing the admission and supported discharge experience for people with complex clinical presentations.
  • Monitor compliance with KPIs and where necessary develop robust action plans to address non- compliance.
  • To assess compliance with CQC standards and where necessary develop robust action plans to bring the service in line to achieve a minimum rating of Good.
  • Develop and contribute to the development of care pathways in conjunction with the ICB and other stakeholders and providers.
  • To promote an integrated working culture across Therapy, Care and Nursing teams to enhance rehabilitation outcomes and the patient experience
  • To work within the ethos of practice development to ensure the teams are mentored in areas of clinical practice and are encouraged and supported to develop clinical and leadership skills
  • To support the team in the implementation of treatment plans to ensure that patients always receive a high quality of care, liaising with partnership organisations to risk manage complex cases
  • To be actively involved in the design and implementation of research and/ or clinical audits/ trials to evaluate that aspect of the service and identify areas for development, maintenance and improvement.
  • To develop and maintain good working relationships and liaison with colleagues and other health professionals regarding patient care i.e. GPs, MKC, UCR, and MKUFHT.
  • To monitor outcome measure data sets to determine service performance and capacity of complexity.
  • To provide oversight monitoring of team capacity, allocation and waiting times to ensure safe service provision and flow across the health and social care system.
  • To manage any service development via the Planned Care Health Development Action Plan (HDAP) or formal project plans as required.
  • To support the successful transition of people with complex needs across pathways from community to acute and returning home, by in-reaching into the acute environment and attending multi-agency meetings as required.
  • Joint working with teams to support clinical conversations / appointments to streamline transition of pathway.
  • To work with the team and other Home 1st colleagues to ensure compliance with the CQC key lines of enquiry and submit evidence of this as requested.
  • To work with colleagues to establish clinical protocols and standards to meet NICE guidance and establish baseline service assessments in comparison.
  • Undertake evidence-based audit and research projects to further own clinical practice and service development. To interpret and implement changes to national professional policies
  • To project a smart, professional image always, therefore reflecting the high standards expected within the department.
  • To maintain professional standards of conduct according to Trust and other regulating bodies.
  • To undertake training and CPD to ensure Professional Registration is kept up to date, and to produce certification when requested.
  • To attend related courses that would complement the work carried out in the role.
  • It is the individual's responsibility to hold and update their own training portfolios, in accordance with C.P.D. professional requirement.
  • To ensure that all mandatory and job specific training identified by CNWL- MK and the department is kept up to date.
  • To monitor and report on KPIs for the different aspects of the service to ensure contracted standards are maintained
