Macmillan Personalised Care Breast Clinical Nurse Specialist

We are pleased to invite applications to join the Breast Clinical Nurse Specialist (CNS) team based at Frimley Park Hospital, Camberley, funded by Macmillan Cancer Support.

More people are living with and beyond cancer. Receiving care that is tailored to a person's needs can significantly impact their experience and quality of life. Providing personalised stratified follow-up is vital for delivering world-class cancer services.

Supported by the Band 8a Cross-Site CNS Team lead, the successful post holder will collaborate with cross-site colleagues to establish a dedicated team to advance the Trust's Supported-Self-Management-Pathway (SSMP) service, also known as Patient Initiated Follow-Up (PIFU), aligning with national cancer strategies to deliver personalised care.

The CNS plays a pivotal role in ensuring an excellent care experience for breast cancer patients during and after treatment. The post holder will also assist the Primary CNS team in supporting newly diagnosed patients and throughout their treatment, gaining experience in care coordination and service delivery, and leading the transition to supported self-management.

The role will be primarily based at Frimley Park Hospital, with a cross-site overview.

Main duties of the job

The successful applicant will establish and develop a comprehensive nurse-led service that is holistic, evidence-based, and tailored to individual patient needs post-treatment. The role requires autonomous working and team collaboration.

We seek a forward-thinking senior nurse motivated to develop quality improvements within personalised breast cancer care. The candidate should be ambitious, dynamic, and innovative in service transformation.

Responsibilities include care coordination with the MDT; delivering nurse-led clinics; providing specialist advice on breast awareness, signs, and symptoms of recurrence; and facilitating rapid re-access to the breast cancer team via telephone support.

There will be opportunities to work with the Macmillan Information and Support Service to develop initiatives like Health and Well-Being Days. Administrative support will be provided.

Salary: £48,270 to £54,931 per annum, including HCAS.
