The Macmillan Health Care Assistant Band 3 is an experienced member of the team undertaking person-centered care in the community setting and supporting the Band 2 MCHT.
This is an exciting opportunity for joining a compassionate team who deliver end of life services. The team works closely with the District Nurses, Community Macmillan Nurses, End of Life Lead Nurse, Unscheduled Care Team and outside agencies when necessary to ensure the best possible care.
The Macmillan Health Care Team provides a robust and consistent package of care for end of life patients and their families seven days a week, 24 hours a day. The service may include afternoon sits and night sits.
The Macmillan Health Care Service is a central referral point for end of life services. The team collaborates with District Nurses, therapists, General Practitioners, Macmillan Nurses, and short-term care teams. The team is passionate about their service and operates under a guiding philosophy. The successful candidate may be expected to support and collaborate with other community teams as needed.

Main duties of the job
Assist with personal care for patients, up to 4 visits daily, including night sits.
Support patients at the end of their life who choose to remain at home, providing compassionate care, supporting families, and respecting patient independence and dignity. Adapt care to individual and family needs, ensuring high standards of care and resourcefulness.
The Band 3 Healthcare Assistant works alongside the community nursing team, undertaking person-centered care and supporting the Band 2 MCHT.
Carry out delegated work in accordance with the National Qualification Framework (NQF) Level 3 and local policies.
Must have access to own car, a valid driving license, and less than six points on the license.
