An exciting opportunity for an Assistant Practitioner within the Manchester Macmillan Palliative Support Service. The role involves working closely with a multidisciplinary team to provide high-quality palliative care for patients with life-limiting illnesses, ensuring they receive support in their preferred environments. Candidates must hold a foundation degree to qualify for this position.
An exciting opportunity has now arisen within the Manchester Macmillan Palliative Support Service for an Assistant Practitioner. Please note the foundation degree is an essential requirement for this post.
The post holder will work within the Macmillan Specialist Palliative Care Team, contributing to the provision of a co-ordinated and comprehensive service for patients receiving specialist palliative care, including patients residing in care homes in South Manchester.
The post holder will work as part of the multi-disciplinary team comprising Doctors, Nurses, Dieticians, Occupational Therapists, Physiotherapists, and Speech and Language Therapists. The team aims to provide effective palliative care and support to patients with any life-threatening illness.
The post holder will visit patients in their own homes or care settings and assist other professionals in the implementation, delivery, and evaluation of palliative care and education programmes to promote independence and improve health, functional capabilities, and quality of life. They will support patients and their families as they approach the final stages of life.
The post holder will hold a small caseload under the supervision of the responsible clinician, supporting patients with life-limiting illnesses to maximise their health and enable them to be cared for in their preferred place.
The post holder will need to demonstrate the ability to be mobile within the community.
