About the Role

The Macmillan 1-1 Primary Care Nurse will support cancer patients in the community, focusing on personalized care and support across Oldham. Responsibilities include conducting Holistic Needs Assessments, developing care plans, promoting self-management, and working with multidisciplinary teams to monitor and support patients, including those with palliative care needs.

Key Responsibilities
  1. Complete Holistic Needs Assessments (HNA) and develop personalized care plans.
  2. Collaborate with patients and families to identify needs and promote healthy lifestyles.
  3. Educate on signs of deterioration and long-term effects of treatment.
  4. Coordinate with generalist and specialist services for patient support.
  5. Support team leadership and multidisciplinary team activities.
