Job description

LS25/26 PCN Healthcare Assistant and Social Prescriber

Monitor and review care plans and agreed outcomes in partnership with the person. Promote the benefits of social prescribing and the work of local groups and communities, including responsibilities to educate other members of the PCN team on available services and support in the wider community. Act as an advocate for the person and report to senior staff when required.

Communication

Provide and receive information with empathy and understanding, adapting communication when barriers to understanding arise, seeking senior guidance when necessary. Develop and maintain effective working relationships with other primary care team members.

Administration

Keep timely, accurate, and complete electronic patient records and activity data, maintained and stored in accordance with professional and organizational standards. Responsible for appropriate documentation related to patients in their care, ensuring dissemination to relevant professionals in a timely manner.

Clinical Duties

Clinical duties may include:

  • Phlebotomy
  • Undertaking CCSP1 and dementia reviews
  • NHS Health Checks
  • Blood pressure, height/weight recording
  • Performing ECGs and urinalysis
  • Chaperoning duties
  • Accurate labeling of samples
  • Checking patient identification
  • Recording sample collection and dispatching samples to laboratories
  • Reporting anomalies and significant patient issues
  • Liaising with administrative team
  • Health promotion and education
  • Signposting and referral to services
Skills and Experience

Must be empathetic, caring, and patient-focused. Experienced phlebotomist with good experience working with elderly patients preferred. Healthcare setting experience, competent in clinical observations, screening, and assessments under supervision. Hardworking, motivated, effective team player, with excellent communication and IT skills, organized, and able to manage time effectively.

Other Responsibilities

Participate in service development discussions, audits, policy implementation, and safety procedures. Engage in appraisal, CPD, mandatory training, mentoring, and sharing best practices.

Confidentiality and Safeguarding

Adhere to policies on confidentiality, data protection, and safeguarding children and adults at risk. Complete required safeguarding training and seek supervision when managing complex cases.
