Overview
The post holder will be a SCPHN Qualified School Nurse who will provide specialist advice and evidenced based practice to clients and schools in Enfield to ensure that children and young people with long term health conditions are well supported and have a safe environment at school. The role influences the work undertaken by the wider school nursing workforce and contributes towards identifying gaps, good practice and service development opportunities for inclusion in the annual workplan for long term conditions. The nurse will lead on the implementation of the annual work plan for long term conditions and participate in partnership meetings to improve Enfield's response to long term conditions, attending meetings and forums to promote initiatives in the annual plan. Initial focus will be on improving the management of childhood asthma by leading on the implementation of an Asthma Friendly Schools initiative in Enfield to support children, young people, families and schools and reduce the number of school days missed due to asthma. Long Term Conditions will develop Enfield School Nursing Service's approach to other conditions (which may include allergies, epilepsy, eczema, sickle cell and diabetes).
To work in partnership with the 0-19 Service Team to achieve Asthma Friendly Accreditation in Enfield Schools and promote health and well-being of Children, Young People aged 5-19 years and their families in the school environment. This will be achieved through evidence based practice, NICE guidelines, Government Directives, NHS Plans, Specialist Nurses, GPs and other Specialist Practitioners, workload management and monitoring of care standards.
Main duties and responsibilities
- Support the 0-19 Service Team to achieve Asthma Friendly Accreditation in Enfield Schools, maintain and update this Accreditation.
- Work in partnership with the 0-19 School Nursing and Health Visiting Teams, service users and all stakeholders to promote the health and well-being of Children, Young People aged 5-19 years and their families. Achieve this through effective delegation, workload management and monitoring of care standards.
- Promote evidence-based practice in the school environment, aligned with NICE Guidelines, Government Directives, NHS Plans, and collaboration with Specialist Nurses, GPs and other Specialist Practitioners.
- Identify and monitor health needs of a given population using profiling tools and facilitate implementation of evidence-based care in line with the Healthy Child Programme and safeguarding policies for children 5-19 years and their families.
- Manage and prioritise workload within a defined caseload, ensure efficient use of resources, maintain accurate data and measure success via audits and Clinical Governance.
- Link with schools and family hubs to promote, advise and guide current best practice/NICE guidelines and initiatives (e.g., Asthma Friendly Schools; Sickle Cell guidance; Anaphylaxis best practice).
- Agree an annual work plan for long term conditions between the Service Lead for School Nursing and the School Nurse for Long Term Conditions to inform the year’s workstreams.
- Recognise priority workstreams based on local need, gaps in provision and required service developments, in partnership with Enfield Council's Public Health Team.
Responsibility for patients and schools/family hubs/local authority/NHS organisations
- Keep contemporaneous notes and accurate records; maintain confidentiality of clients/schools/family hubs according to NMC and Trust guidelines.
- Work within the policies and guidelines set out by the Trust and the NMC Code of Professional Conduct.
- Adhere to all Trust policies, procedural guidelines and current legislation.
- Remain up to date with internal training and development.
- Regularly write reports and collect current data regarding progress and changes in the community; communicate with service users in a caring, professional manner.
Policy and service development
- Undertake such other duties as reasonably required and consistent with the post’s responsibilities.
- Participate in research/audit and statistical data collection as approved.
- Contribute to policies and the development and implementation of care pathways.
- Participate in developing practices and protocols and deliver evidence-based practice to ensure high standards of care; contribute to audits and quality improvement projects.
Financial and physical resources
- Ensure the correct use and storage of resources.
- Maintain equipment and report malfunctions to senior staff; be aware of Health and Safety hazards and follow Trust policies accordingly.
- Ensure all equipment is accounted for and maintained per Trust instructions.
Leading and managing
- Supervise and delegate duties of support staff; monitor and analyse client and staff needs.
- Adjust work patterns to meet service needs and prioritise issues; provide day-to-day supervision and line management of junior staff with support and professional guidance.
- Car driving for liaison with agencies, attending meetings and training.
- Collaborate as a team player to share and reflect nursing expertise and current best practice.
Information resources
- Maintain excellent computer skills and undertake IT training to develop skills in electronic record keeping and data collection.
- Keep up to date with internal training and development; regularly write reports for case conferences and other meetings as appropriate.
- Maintain own record keeping on the patient record information system (RIO).
Research and development
- Participate in research/audit and statistical data as approved.
- Contribute to planning and implementing effective health promotion and health education programmes for the client group within communities to support Public Health Outcomes.
- Stay up to date with NICE guidance and how to implement it into day-to-day clinical practice.
