Job summary

The post holder will be a SCPHN Qualified School Nurse, who will provide specialist advice and evidenced based practice to Clients and Schools in Enfield to ensure that Children and Young People with Long Term Health conditions are well supported and have a safe environment at school.

Influencing the work undertaken by the wider school nursing workforce.

The School Nurse for Long Term Conditions will be expected to contribute towards identifying gaps, good practice and service development opportunities for inclusion in the annual workplan for long term conditions.

The School Nurse for Long Term Conditions will be the nurse leading on the implementation of the annual work plan for long term conditions. Will be expected to participate in partnership meetings to improve Enfield's response to long term conditions and attend meetings and forums to promote initiatives in the annual plan.

The School Nurse for Long Term Conditions will initially focus on improving the management of childhood asthma by leading on the implementation of an Asthma Friendly Schools initiative in Enfield. The Asthma Friendly Schools initiative will support children, young people, families and school to improve the management of asthma and reduce the number of schools days missed due to asthma.

Long Term Conditions will develop Enfield School Nursing Service's approach to other conditions which may include allergies, epilepsy, eczema, sickle cell and diabetes.

Main duties of the job
  • Support the 0-19 Service Team to achieve Asthma Friendly Accreditation in Enfield Schools.
  • To work in partnership with the 0-19 School Nursing and Health Visiting Teams, service users and all stakeholders in promoting the health and well-being of Children, Young People aged 5-19 years and their families.
  • Identify and monitor health needs of a given population using profiling tools and facilitate the implementation of evidence-based care.

The 0-19 North Mid in the Community is an evidence-based service who support families, children and young people in the community. This is via the Healthy Child Programme, professional development and NICE guidelines.
