Job description

We are looking for a Long Term Conditions Nurse to join our team. The role is part time working 22.5 hours per week. The role includes working on site with face to face and telephone consultations, plus the option for some home working. There is some flexibility with the working pattern.

Main duties of the job

To provide and maintaina high standard of nursing care for patients as well as providing nursingassistance to the doctors and other members of the primary healthcareteam.

The duties will include all tasksnormally undertaken by an experienced RGN and in addition any roles agreedbetween the nurse and the doctors as appropriate, having regard to currenttraining.

Job responsibilities
  • To support the delivery of the AgeingWell programme by delivering clinical care to patients and working inpartnership with other PCN staff to ensure the program is successful.
  • Assist in and perform routine tasksrelated to patient care
  • New Patient Medicals recognise healthpromotion opportunities and refer to clinic/colleagues where appropriate
  • Work closely with a range of internaland external partners to deliver the Ageing Well program.
  • Work with other health careprofessionals to monitor and manage the treatment of Chronic Diseases.
  • Conduct respiratory, Cardiac anddiabetic assessments and reviews.
  • Review medication for therapeuticeffectiveness appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols for patients withChronic Disease.
  • Provide information and advice onprescribed or over the counter medication on medication regimes, side effectsand interactions. Prescribe when necessary.
  • Running well person clinics and healthpromotion programmes by providing a holistic assessment and lifestyle advice ondiet, smoking, alcohol intake and exercise.Facilitate change. Give briefinterventional support and refer as appropriate.
  • Work professionally carrying out allduties commensurate with the role and in line with NMC Code of ProfessionalConduct and the Scope of Professional Practice
  • Keep up to date with mandatory trainingincluding safeguarding and data confidentiality etc.
  • Apply infection control measures in thepractice according to the practice/local/national guidelines.
