The successful candidate will be a Registered Nurse from either Acute & Community or Mental Health backgrounds, eager to develop assessment skills further in an autonomous role within the NHS Long Term Care Service. This role emphasizes multidisciplinary working and requires understanding of policies related to Continuing NHS Healthcare, NHS Funded Nursing Care, and the Social Services and Well-being (Wales) Act 2014.
Hywel Dda University Health Board offers a dynamic and innovative environment. We have invested in the NHS Long Term Care Service and introduced a new person-centered care model. We are recruiting highly motivated, patient-focused Long Term Care Specialist Nurses to support patients in care homes and staff as part of a Nursing Home Support Service. Our nurses also support secondary care through a newly developed Long Term Care Pathway, facilitating smooth transitions from acute episodes to discharge and nursing home placement.
We seek an enthusiastic, experienced nurse with excellent assessment and communication skills who enjoys challenges. The role involves working within the Nursing Home sector, assessing and monitoring individuals, providing training and support to nursing homes, and reviewing patients for Continuing NHS Healthcare and Funded Nursing Care. The role also includes managing multidisciplinary team meetings related to funding and discharge planning.
The successful candidate must demonstrate excellent communication skills and the ability to work both independently and as part of a team. Responsibilities include assessing individuals' care needs, monitoring standards of care in nursing homes, and ensuring appropriate funding.
The base can be in Carmarthen, Llanelli, or Pembroke Dock, with potential for change. Travel across the Hywel Dda area and neighboring counties is required.
Welsh language skills are desirable; applications from English and Welsh speakers are equally welcome.
