Job summary

FutureCare is seeking a Physical Therapist to evaluate and treat movement dysfunction, restore mobility, and create individualized care plans for residents in Maryland.

Qualifications

• Ability to assess resident needs and develop and implement a comprehensive plan of care.
• Knowledge of accreditation standards and compliance requirements.
• Strong documentation and communication with care teams.

Responsibilities

• Review available resident information and evaluate impact on assessment and treatment.
• Conduct and document thorough functional assessments.
• Develop and implement individualized treatment plans with achievable goals.
• Complete re-evaluations and supervise staff when appropriate.
• Document course of treatment and prepare discharge summaries.
• Educate residents and families on exercise programs and procedures.
