Overview

An exciting opportunity has arisen for one full time locum Specialist Doctor in Psychiatry with our H&F North and South Mental Health Integrated Network Teams (MINT) for six months with potential to extend to one year. South MINT -6 PAs working based at Claybrook Centre; North MINT- 4PA working based at Fulham Palace Road.

The MINT model is aimed to better meet individual needs with MDT by developing good relationship with the local Primary Care Network (PCN). This is a transformed service and the workload assessment is based on current patterns of referrals as well as existing caseloads combined. There are 5 PCNs in Hammersmith and Fulham and each of these is supported by an MDT. One whole time Consultant Psychiatrist is allocated to each of the PCNs. Within each team, there is a whole team approach to caseloads. The post is created as there is a 6PA vacancy unfilled in South MINT and the need to backfill 4PA in North MINT as the team consultant has taken up the clinical lead of the service.

You will have a key role in a liaison/consultation model to provide early, easy access advice to GPs and the Primary Care Networks to support care of patients in the locality. GPs in the borough will be able to contact you via telephone or email for advice during working hours. You will also provide clinical leadership to the team in clinical multi-disciplinary team meetings and referrals meetings as well as being available for case discussions, advice and supervision within and outside team meetings. You will also have a role in the management of more complex cases, to offer senior assessments and more direct input in particular in risk and/or medication management and diagnostic assessments. This will also include Mental Health Act Assessments, CTO reviews and supporting the duty function of the team with urgent concerns and assessments. You will also share senior medical responsibility with job sharing consultant for the care carried out by the MINT team.

There is an indicative of 7 outpatient clinics per week with mixture of new and follow clinics.

  • Facilitate smooth transitions into and out of the service through relationships with other relevant services.
