Based inTruro city centre, serving over 18,000 patients, this is an exciting opportunityfor an experienced practice nurse to contribute tothis busy practice. This post will be ideal for someone who is passionate about supporting patients who are living with a long-term condition, and who has at least two years post qualification experience in managing long-term conditions.
We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisation and communication skills.
Practice Nurse experience is essential as is a minimum of two years post-registration experience of providing nursing care to patients with diabetes. We welcome experience in other long-term conditions, including asthma, COPD, and cardiovascular disease.
The role requires resilience and the ability to be flexible and adaptable. The successful applicant will be working alongside GPs and experienced nurses to offer care, advice, support and education to patients with long term conditions. Working both independently and with our other chronic disease nurses, you will primarily deliver care to patients who have been diagnosed with diabetes (and other long term conditions e.g. asthma, COPD, cardiovascular disease) as well as undertaking those duties usually undertaken by a Practice Nurse.
TheThree Spires Medical Practice operates from Truro Health Park and we have adedicated team of GPs, nurses, phlebotomists and physiotherapists caring forover 18,000 patients. We are a learning practice, hosting GP trainees, medicalstudents and nursing students from the Peninsula School of Primary Care, theUniversity of Exeter and Plymouth University.
The post holder will undertake a range of nursingassessments and provide appropriate care / treatment in conjunction with GPs,other Practice Nurses or independently as appropriate for the need, accordingto Practice policies and protocols. Thepost holder will strive to improve standards of care and be involved inidentifying Practice population needs.
