Main duties of the role include:
Providing clinical care in the treatment room, including wound care, injections, and immunisations.
Supporting the management of long-term conditions such as diabetes, asthma, and hypertension.
Performing routine health checks, screenings, and vaccinations.
Working collaboratively with the Lead Practice Nurse, GPs, and the wider multidisciplinary team.
Contributing to the development and improvement of primary care services in response to increasing demand.
We are looking for individuals with proven Primary Care nursing skills and experience, who can demonstrate safe, evidence-based clinical practice, excellent communication, and a commitment to patient-centred care.
Provide high-quality nursing care to patients in a primary care setting, including managing long-term conditions such as diabetes, asthma, and hypertension.
Conduct routine health checks, screening, immunisations, and travel vaccinations, as well as treatment room procedures including wound care and minor procedures. Support patient self-management through education, advice, and health promotion.
Work collaboratively with GPs, the Lead Practice Nurse, and the wider multidisciplinary team to ensure safe, effective, and patient-centred care.
Contribute to service development and quality improvement initiatives, helping to shape and improve the patient experience at the practice.

This position will involve working arrangements across Embrace Medical Centre and Martlesham Heath Surgery. As part of our joint partnership, both practices are operated by the same GP Partners and Management Team. The surgeries are located approximately 2 miles apart (around a 5-10 minute drive depending on traffic), ensuring close collaboration and easy access between sites.
