Job summary

Locum Ready is seeking a qualified Practice Nurse to join a GP practice in Hounslow on a locum basis. The role focuses on chronic disease reviews, wound care, immunisations (where trained) and routine clinic activity within a structured primary care environment.

You will manage recalls, health checks and treatment room duties, contributing to high-quality patient care with clear clinical documentation and adherence to safety guidelines.

Qualifications

• NMC-registered nurse with experience in primary care.
• Immunisation training desirable.
• Confident in chronic disease management and patient care.

Responsibilities

• Conduct chronic disease reviews for patients with asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out adult and children’s immunisations (if trained).
• Undertake smear tests (cytology-trained nurses).
• Complete NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.

This role is ideal for a Practice Nurse with strong assessment skills who can confidently deliver chronic disease reviews, treatment room procedures and routine nursing clinics in a structured primary care environment.

You will contribute to high-quality patient care through a balance of clinical reviews, preventative health work and treatment room activity. The practice offers well-organised workflows, supportive nursing leadership and a clear structure for daily clinic delivery.

Key Responsibilities

Requirements
