Job summary

Evolve Staffing Ltd is seeking an adaptable General Practitioner (GP) for an ongoing primary care post based in Gosport, Hampshire. The appointed doctor will deliver general medical services, face-to-face consultations, telephone triage, and chronic disease management to a diverse local patient population, supporting the practice and local Primary Care Network (PCN).

Qualifications

• GMC Registration and current Licence to Practise.
• Inclusion on the NHS England Performers List.
• MRCGP or an equivalent recognised primary care qualification.
• Demonstrable NHS general practice experience.
• High proficiency in primary care software (EMIS Web or SystmOne).

Responsibilities

• Deliver face-to-face clinical consultations, home visits, and remote triage (phone/video).
• Manage complex chronic conditions and care plans per clinical standards.
• Prescribe safely using the practice's EMIS Web or SystmOne system.
• Process lab results, hospital discharge summaries, referrals and coding.
• Participate in safeguarding, palliative care reviews and audits.

The appointed doctor will deliver general medical services, face-to-face consultations, telephone triage, and chronic disease management to a diverse local patient population, supporting the practice and local Primary Care Network (PCN) in delivering outstanding patient outcomes.

Core Clinical Responsibilities:

• Deliver face-to-face clinical consultations, home visits, and remote telephone or video triage for acute and chronic medical presentations.
• Manage complex long-term conditions, chronic disease monitoring, and multi-morbidity care plans in accordance with Quality and Outcomes Framework (QOF) clinical standards.
• Prescribe medications safely and appropriately using the practice's electronic clinical system (EMIS Web or SystmOne), adhering to local prescribing formularies.
• Process clinical administration efficiently, including electronic laboratory results, hospital discharge summaries, clinical coding, and medical referrals.
• Participate in multi-disciplinary safeguarding meetings, palliative care reviews, and clinical practice audits.

Essential Candidate Requirements:

• Full GMC Registration with a valid Licence to Practise.
• Inclusion on the NHS England Performers List.
• MRCGP qualification or equivalent recognized primary care qualification.
• Demonstrable clinical experience delivering primary care medical services within an NHS general practice environment.
