Job summary

BrisDoc seeks a SevernSide IUC Self-Employed GP to manage urgent primary care presentations, providing telephone, video or face-to-face consultations, and prescribing when needed, including end-of-life care. You will document thoroughly to ensure continuity of care and contribute to peak-demand rotas.

You will mentor new clinicians and collaborate with the BNSSG urgent care network, supporting patients across the Severnside Integrated Urgent Care and related services.

Qualifications

  • Maintain thorough documentation for continuity of care.

Responsibilities

  • Assess and manage the full breadth of urgent primary care presentations.
  • Take telephone referrals for appropriate adult medical admissions.
  • Demonstrate excellent communication with referring HCPs to foster collaboration.
  • Gain familiarity with BNSSG urgent care services and access routes.
  • Undertake clinical work by telephone or video, assess concerns, and formulate management plans.
  • Support community management while recognising serious illness requiring admission or ambulance response.
  • Be able to prescribe appropriately for urgent primary care contexts, including end-of-life adjustments.
  • Document key findings to log assessments and aid subsequent clinicians.
  • Seek clinical advice from the senior GP on shift when required.
  • Use IT systems for safe clinical work in IUC (CLEO, PACS, EMIS) and other BrisDoc resources.
  • Proactively report concerns and incidents to support service improvement.

Job description

As a SevernSide IUC Self-Employed GP your role will involve, managing a wide range of clinical presentations in urgent care settings, supporting patients via telephone, video, or face-to-face consultations, prescribing effectively, including adjustments to end-of-life care, maintaining thorough documentation for continuity of care, as well as mentoring new clinicians and contributing to peak-demand rotas.

Main duties of the job
  • Assess and manage the full breadth of clinical presentations and age groups which present in the urgent primary care setting, including physical and mental health needs
  • Take telephone referrals from Health Care professionals within the BNSSG area for appropriate adult medical admissions whilst being sensitive to the capacity issues of the hosting acute trust
  • Demonstrate excellent communication skills with referring HCPs, in order to build positive relationships so that alternative management ideas can be discussed without confrontation
  • Gain familiarity with the other urgent care services available in the BNSSG system, and how to access these for patient
  • Undertake clinical work by telephone +/- video, consulting with patients and/ or their representatives and healthcare professionals in the community to appropriately assess the current concerns, establish an appropriate differential diagnosis and management plan.

  • Seek to support community management wherever possible, but be able to recognise serious illness and emergency situations requiring admission and/ or an ambulance response
  • Be able to prescribe appropriately for the urgent primary care context, including initiating and changing end of life medication
  • Document the key findings from their assessment, such that the notes provide appropriate detail to log the assessment at the time, and support other clinicians who may be involved later in the patients journey
  • Seek clinical advice from the senior GP on shift (the Clinical Coordinator where applicable) when required to support clinical decision making and access to appropriate service
  • Use all IT systems required for safe clinical work in IUC, including the clinical systems (CLEO, PACS and EMIS), resources available to support clinical decision making (the Clinical Toolkit, BrisDoc weblinks, Remedy
