Description: Part-time Performer Dentist required for 2-3 days per week (MON, THURS & FRI) in a well-established practice near Bootle in Liverpool.
• £500 per day
• Monday, Thursday & Friday ONLY
• Fully computerised surgeries - Spacious 4 surgery practice
• OPG, Digital X-rays, Rotary Endo
• Separate decontamination room
• Full support team including a hygienist/therapist
• Practice Hours - Monday to Friday: 9am-5.45pm

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
